To help patients achieve optimal health outcomes, health systems need to identify and address the social determinants of health (SDOH) – the conditions under which people are born, grow, live, work and age. This is becoming of greater interest as health care organizations are increasingly held accountable for reaching population health goals while reducing costs. It is important that they have tools and strategies to identify the upstream socioeconomic drivers.
